Aim: to evaluate the efficacy and safety of 0.09% bromfenac eye drops in the comprehensive management of postoperative inflammation and prevention of pseudophakic cystoid macular edema (PCME). Patients and Methods: the study included 143 patients (152 eyes) who were divided into study and control groups. The study group consisted of 71 patients (79 eyes), while the control group included 72 patients (73 eyes). All patients were examined on postoperative days 7 and 14 after uncomplicated phacoemulsification. Optical coherence tomography was performed preoperatively and during postoperative follow-up in all patients. Postoperative anti-inflammatory therapy and PCME prophylaxis in both groups included instillation of a fixed combination of levofloxacin and dexamethasone four times daily for 7 days, followed by dexamethasone 0.1% eye drops four times daily from postoperative days 8 to 14. In addition, patients in the study group received 0.09% bromfenac eye drops twice daily for 4 weeks starting from postoperative day 1. The primary endpoints were the degree of anterior segment inflammation and the absence of PCME 6 weeks after phacoemulsification. Secondary endpoints included the incidence of acute endophthalmitis, ocular surface status, drug tolerability, and patient-reported ease of use of the auxiliary eye drop instillation device (strike‑up support) supplied with the study medication. Results: no cases of acute iridocyclitis or endophthalmitis were observed in either group. Absence of postoperative inflammatory reaction in the study group was documented in 93.59% and 96.15% of cases on postoperative days 7 and 14, respectively, compared with 91.8% and 95.9% in the control group. PCME developed in 2 eyes in the study group and in 4 eyes in the control group (p>0.05). Good tolerability and safety of 0.09% bromfenac eye drops were reported in 97.27% of patients. The strike-up support device was rated as convenient by 90.14% of patients. Conclusion: the results demonstrate the high efficacy and safety of 0.09% bromfenac eye drops in the comprehensive management of postoperative nonspecific inflammation and in the prevention of PCME.
