It was shown that intraosseous granulomas (similar to granulomas which develop in soft tissues) are the pathomorphologic substrate of the foci of destruction which can be detected radiologically in different parts of the skeleton of patients with rheumatoid arthritis. Intraosseous granulomas initially have a necrotic nucleus, sometimes with fragments of necrotizing osseous structures, surrounded by a typical cellular zone. Subsequently they become encapsulated and transform into fields of delicate fibrous connective tissue, and against their background new foci of inflammation may develop; sometimes their decomposition occurs. Intraosseous rheumatoid granulomas of macroscopic size are detectable radiologically. Characteristic features of their structure that can be used for assessment of the extent of an allergic reaction of the bone tissue are given.
