N2 - Background: To estimate the efficacy and cost of the modified immunochemiluminescent quick PTH assay using the decreased reagents incubation time. Methods: Three hundred twenty-six patients with primary hyperparathyroidism were operated in 2011–2012 using the modified PTH assay. In all the cases, selective parathyroidectomy was performed with PTH testing in three time points: pre-incision, pre-excision, and 10 min after adenoma removal. Blood samples were estimated by two methods: standard PTH assay (DiaSorin Liaison, Italy, time to result 25 min) and modified assay (incubation time decreased from 20 to 10 min, total time to result 15 min). In a modified test, the analyzer algorithm was changed to stop the incubation phase at 10 min without the full conjugation of the reagents and proceed to a washing phase and then to a reading phase. Results: False positive and false negative results were revealed in 1.2 and 11.4 % of cases using modified test and 50 % drop cut-off level. With 40 % drop cut-off level, the
