When correcting complex deformities, a hexapod frame is advisable. When correcting multi-apical deformities, the hexapod frames are mounted at the level of each osteotomy. This increases the bulkiness of the frame and makes it more difficult to use the hexapod software. The “Spring Technique” (ST) provides the use of only one orthopedic hexapod (six struts) for all apexes of the deformity and greatly simplifies the calculations of deformity correction. When mounting the frame, the struts are fixed to the proximal and distal rings. The intermediate rings are fixed with springs. Before correcting the angular deformity, it is imperative to gradually ensure a diastasis of 5 mm at the level of each osteotomy! When using the software, the axes of the proximal and distal fragments are taken into consideration. Correction of intermediate fragments is done “automatically” by means of the springs. This case demonstrates the advantages of ST in the correction of multilevel deformities of the lower leg and femur. On the lower leg, the correction period was 63 days, and the fixation period was 310 days. On the right femur, the correction period was 26 days, and the fixation period was 260 days. There was no option for transition from external to internal fixation. A good anatomical and functional result was obtained. The patient expects the next stage: ST-based correction of the deformity of the left lower extremity. © Springer Nature Switzerland AG 2015, 2024.
