It is important to establish prognostic factors for the course of the disease in patients with the first episode of schizophrenia. The aim of the study was to establish predictors of therapeutic response and remission in patients with the first episode of schizophrenia. Material and methods. The study included 92 patients aged 24.1±5.1 years with the first episode at the stage of inpatient treatment and in the dynamics for 2 years with the use of clinical assessment, PANSS. Statistical data processing was performed using the Statistica for Windows version 8.0 software package. Results. Initial admission to the hospital the severity of psychopathological symptoms was consistent with moderate severity: a general indicator of PANSS amounted to 108.6±16.1 points. Despite the relatively short duration of the disease in patients were significantly expressed not only productive (27.5±5.35 points on PANSS), but also negative (26.2±5.06 points) symptoms. Therapeutic response and a significant reduction of psychopathological symptoms were achieved in all cases, regardless of the type of antipsychotic therapy. The time of onset of therapeutic response - 5.7±2.6 weeks. In half of patients, the degree of reduction of the total index was in the range of 30 to 50% (46 patients, 50%), which corresponded to a moderate therapeutic response. In general, the treatment process was able to achieve a reduction of the total score on the PANSS is 46.5%, with the largest mobility found for positive symptoms. In patients with the first psychotic episode, the paranoid type of remission prevailed (53.2% of patients). Within two years, all patients diagnosed with “Acute polymorphic disorder with symptoms of schizophrenia” F23.1 were re-hospitalized and the diagnosis was changed to”Paranoid schizophrenia” in 90.6% of cases. Recurrence within two years occurred in 53.3% of patients, while among patients, in which the reduction of the total score by PANSS was less than 30%, the relapse developed most frequently, and the recurrence rate in patients with good and medium therapeutic response did not differ significantly. The mobility of psychopathological symptoms in the process of cupping therapy in patients who subsequently relapsed was less. In patients with remission during the next two years, the severity of negative symptoms before treatment was higher than in patients with relapse. Conclusion. The nature of the therapeutic response is of prognostic importance for assessing the risk of relapse in patients with the first episode of schizophrenia.
