INTERLEUKIN-18 AND RENAL ANEMIA IN PATIENTS WITH TYPE 2 DIABETES MELLITUS AND EARLY STAGES OF CHRONIC KIDNEY DISEASE

INTRODUCTION: Inflammation is involved in the pathogenesis of complications of diabetes mellitus, including diabetic nephropathy and renal anemia. In this study we aimed to assess clinical correlates of serum interleukin-18 (IL-18) levels in anemic and non-anemic patients with type 2 diabetes (T2DM) and early CKD.

METHODS: We investigated 65 patients with T2DM complicated with CKD stages 1-3 (30 anemic and 35 non-anemic individuals). Anemia was defined according to WHO definition (2008). Patients with non-renal causes of anemia were not included. eGFR was calculated using CKD-EPI formula. In addition to routine clinical tests we measured serum levels of NT-proBNP and IL-18. Mann-Whitney U-test and Spearmen’s correlation coefficient (rs) were used for statistical analysis.

RESULTS: We found that elevated levels of IL-18 were more prevalent in anemic patients than in individuals with normal levels of hemoglobin (20.0% vs. 2.9%, p=0.023, respectively), while there were no significant differences in eGFR, NT-proBNP, left ventricular mass index and ejection fraction between the groups. In anemic patients IL-18 correlated positively with NTproBNP (rs=0.421, p=0.036), ferritin (rs=0.430, p=0.032), and albuminuria (rs=0.479, p=0.020). However, no similar interconnections were found in patients with normal hemoglobin level. In both groups IL-18 concentration was not interconnected with age, body mass index and eGFR. The subgroup of patients with elevated ferritin levels (n=15) was characterized by strengthened correlation between IL-18 and albuminuria (rs=0.631, p=0.015).

CONCLUSIONS: The results of the study suggest that IL-18 could be involved in the pathogenesis of diabetes-related CKD and renal anemia in patients with T2DM. Serum concentration of IL-18 is interrelated with albuminuria and serum levels of NTproBNP and ferritin in anemic patients with T2DM and early CKD, but not in individuals with normal levels of hemoglobin.
